List of Items Needed for Application Process: Proton Treatment
 
First and foremost, you’ll need to get an appointment with a proton center for a consultation and have your child become a patient so when the time is right, treatment can begin without any problems or hold-ups. Before treatment will begin, most proton centers will require having all the medical information (records/ papers) to review the case and determine if proton is appropriate for treatment. You will need copies of all medical information to be sent to the proton center for review. Although there are several methods of getting the medical information to the centers; mail, fedex/ ups, etc… the best and quickest way, same day, is to have all the copies made into a PDF file and sent electronically via e-mail. This usually can be done at home, office or any copy center stores. 
 
This information should include copies of:
 
  • Pathology report(s)
  • Operation/ surgical report(s)
  • Physician and/ or Progress notes
  • Recent lab report(s)
  • Road map of chemo plan
  • Imaging study report(s) and CD’s of any MRI, PET scan, CT scan, x-ray(s), etc…
  • Summary of Child's Medical History 
Also needed, but not included in the medical copies, are:
·         Copy of Insurance card(s) front and back

·         Copy of driver's license
